We are seeking for a Registered Nurse to manage all aspects of the intake process, coordinating home care services for all clients, which includes establishing and maintaining the patient’s plan of care, establishing and maintaining positive relationships with clients and referral sources. Responding to clients concerns, and managing the insurance verification and authorization processes.

Receives case referrals. Reviews available patient information related to case, including disciplines required, to determine home care needs.

Obtains initial authorization for services and seeks reauthorization for ongoing and additional services.

Reviews patient’s clinical diagnosis, medications, procedures and clinical course.

Collaborates with the hospital’s discharge planning personnel, utilization review department and insurance Case Managers to facilitate safe, early discharge.

Maintains liaison relationship with hospital and insurance personnel, providing information and education on Organization services, coverage issues and related areas.


Registered nurse with current licensure to practice professional nursing in the state.

At least 2 years experience in health care managed care coordination, preferably in home health care.

Computer literate & experience with SOS software a plus.

Complies with acceptable professional standards and practice.

Ability to market aggressively and deal tactfully with customers and the community.

Demonstrate good communication, negotiation, and public relations skills.
